League Application Form

Please complete the form and return to the League Secretary before 14th June 2017.
At the AGM a list of teams wishing to play next season is given to all attendees. A partially completed form is better than no form. Payment by cheque is the preferred option for the League. All cheques to made payable to 'Mid Norfolk Youth League' or 'MNYL'.
