Application form for 15-16 season

AGM 29th June 2015. Return form by 21st April.

The application form MUST be returned to the League Secretary by June 21st 2015. A cheque made payable to Mid Norfolk Youth League for the correct amount MUST also be sent with the application form. The second part of the form MUST be completed as far as possible and returned with the application form.
