Referees Match Report Card to be completed after every game

The Referee Match Report card can be completed online using the above link, or by downloading and filling in a paper form.

The paper form must be completed then either scanned or photographed and emailed to the league at bdfccards@yahoo.com within 3 days of the game you officiated over.
