Please complete form in advance and bring along on the day. Players do not have to been registered with the league last season.
Maximum of 10 Players per team at U10s, U11s, U12s and U13s and 9 players for U14s, U15s and U16s.
Players can only play for 1 team where Clubs have entered 2 teams at any age group.
